RNE FORMS
GENERAL FORMS, forms collected periodically:
- Type 0-1 Form, reported every month
- Orthopaedic Service Form, periodic reporting
- Orthopaedic Doctor Form, periodic reporting
The forms contain details about orthopaedic service general activity, contact information and available medical equipments.
The forms contain also details about orthopaedists.
HIP FORMS, individual pacient form (3 types):
- Primary hip surgery (Type 2a), collected from september 2001
- Revision hip surgery (Type 3a), collected from september 2001
- Soft tissue hip revision, collected from january 2003
They contain details about the patient and the type of the implanted or revised endoprosthesis.
KNEE FORMS, individual pacient form (3 types):
- Primary knee surgery (Tip 2b), collected from january 2003
- Revision knee surgery (Type 3b), collected from january 2003
- Soft tissue knee revision, collected from january 2003
They contain details about the patient and the type of the implanted or revised endoprosthesis.
SPINE FORMS, individual pacient form (2 types):
- spine form, collected from october 2003
- spine follow up form, collected from octombrie 2007
They contain details about the patient and the type of spine surgery.
CRUCIATE LIGAMENT FORMS, individual pacient form (2 types):
- primary surgery for the reconstruction of anterior / posterior cruciate ligament (Type 5a), collected from october 2014
- revision surgery for the reconstruction of anterior / posterior cruciate ligament (Type 5b), collected from october 2014
They contain details about the patient and the type of the implanted or revised endoprosthesis.
